Older adults are among the fastest growing demographics in the U.S. and are increasingly being served by health centers across the country. As this population ages, most prefer to remain in their communities instead of moving to a residential facility to receive any needed services or support. In this asynchronous activity by the National Center for Health in Public Housing (NCHPH) and the National Center for Equitable Care for Elders (NCECE), we will explore how health centers can identify and address some of the social and environmental risk factors that determine the success of aging in place and quality of life for their older patients.
